Lock Out / Tag Out (LOTO) — also called energy isolation or hazardous energy control — is the safety procedure used to ensure that dangerous machines are properly shut off and cannot be started up again before completing maintenance or servicing work. Uncontrolled release of hazardous energy causes hundreds of Canadian workplace injuries and dozens of fatalities every year — most involve workers being crushed, electrocuted, or amputated by equipment that unexpectedly reactivated during service. Under CSA Standard Z460 and BC OHS Regulation Part 10 (De-energization and Lockout), employers must implement LOTO procedures and train workers.

Effective LOTO requires identifying ALL forms of energy present in a system — not just the obvious ones. Missing a single energy source can be fatal.
To return to service: reverse the process, verify all workers are clear, remove locks (each worker removes their own), notify affected employees, restore energy.
The most important rule of LOTO: every worker who could be exposed to hazardous energy must apply their OWN personal lock, and only they can remove it. Never let someone else lock out for you. Never let someone remove your lock. If workers share locks or one person locks out for a crew, individual workers are left unprotected. Group lockout procedures use multi-lock hasps so every worker maintains individual control.
